This product is listed as a Prescription Only Medication however a school can order a small supply upon submission of a signed order form from the School Head Teacher. To order simply fax, email or post us a fully completed order form (see right) from the School Head Teacher. Please ensure the following has been completed before returning to us:
- Name of the school for which the product is required
- Purpose for which that product is required
- Total quantity required*
